The doctors welcome new patients who live within our practice area. In order to register at this practice you will need to provide two forms of identification, one of these must be photographic, for example a passport or your driving licence and a utility bill with your address on and your NHS number.
As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered an appointment with a healthcare assistant.

Registering with the surgery
To register with the surgery:
- complete the NHS: register with a GP surgery: Macklin Street Surgery form online
Alternatively you can:
- fill out a new patient registration form (word doc) – over 16
- fill out a new patient registration form (word doc) – under 16
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
